and Disability Assistance (OTDA) and
your local Department of Social Services
(DSS), we would like to thank you for
your participation in the HEAP Oil Buying
Component.
In acknowledgement of your commitment
to serve low-income customers, NYSERDA
is offering your company access to the
Heating System Clean & Tune Project.
For each county in which your company
participates in the HEAP Oil Buying
Component, you may be eligible to provide
Clean & Tune services
at no charge to the HEAP customers who
are in need of such service.

If you decide to take advantage
of this opportunity, please print and sign
two (2) copies of the Participation Agreement
and return both copies directly to NYSERDA
at the address shown at the bottom of the
Agreement. The Participation Agreements
will then be signed by NYSERDA, and one
signed original will be returned to you.
The Community Power Network (CPN) will be
managing the Clean & Tune Project for
NYSERDA on a day-to-day basis and will serve
as your contact should you have any questions
or concerns.

Agreement. Your participation will help
improve the heating system efficiency of
the low-income households provided this
service.
Summary of NYSERDA Clean & Tune
Project (July 2008-June 2009):
- NYSERDA will pay for Clean and Tune services (C&T's) you perform for eligible customers up to the number authorized on the last page of your Participation Agreement.
- NYSERDA will pay your normal charge for performing each C&T up to a maximum of $160. You may adjust your charge up to the maximum if program requirements exceed your normal procedure.
- In addition, NYSERDA will pay for minor parts and repairs that may be required beyond the standard C&T, up to the overall limit authorized on the last page of your Participation Agreement. You may use these funds to make justifiable repairs costing up to $300 beyond the cost of the C&T for any one system.
- You must get prior approval from NYSERDA's program representative, Art Godin, to perform repairs to a single system exceeding $300. Such approvals usually require only a short telephone conversation. Total repairs to all systems combined may not exceed the overall company repair limit authorized on the last page of your Participation Agreement without prior written permission from NYSERDA.
- You choose the households to be served among your previous season or current season HEAP customers based on criteria stipulated in the Participation Agreement. You may not choose customers already under a service contract.
- You must perform the C&T according to the specifications outlined in the current version of the Clean & Tune Checklist and Certification Form (Attachment A) and the Participation Agreement. NYSERDA's program representative is available to speak with service managers and technicians about the details of these requirements.
- HEAP recipients
that are renters require landlord approval
for C&T services. The Rental Property
Agreement (Attachment B) serves to explain
the program to landlords and obtain their
approval. You must insert your company
name and address on the form.
- You must submit a fully-completed Checklist (Attachment A) for each C&T service performed and when applicable, a Rental Property Clean & Tune Agreement (Attachment B), in a timely manner. You may request payment for one or several C&T?s on a single Payment Request Form (Attachment C1 or C2).
- NYSERDA offers generous incentives toward the purchase of qualified digital combustion analyzers. Refunds may total up to $500 per unit and up to $2000 per company, following the completion of all requirements specified in the NYSERDA Equipment Purchase Incentive Procedure (Attachment D). Companies who reach the $2000 limit are not eligible for additional funding without written approval from NYSERDA.
- NYSERDA
will reimburse 20% (up to $200) of
the cost of a qualified Combustion
Analyzer* upon proof of purchase;
- An additional
$100 incentive will be paid if the
unit includes a printer;
- An additional
$200 incentive will be paid upon documentation
of 10 Clean & Tune services completed
with this equipment.
*Qualified Combustion
Analyzers must measure Oxygen, Air Temperature,
Stack Temperature, and Carbon Monoxide (CO),
and must calculate Combustion Efficiency,
Excess Air and Carbon Dioxide (CO2).
